Did some more research into that ribbon and found out that the War Merit (w/swords) was for being a combat wounded WWI vet. He did operate on some serious dudes, but unfortunately I'll never hear most of the stories.

If I'm reading it right L-R, War Merit Cross with Swords, 40 years state service (yikes), War Merit Medal 3rd Class.
Working for the state for 40 years..maybe he operated on someone quite interesting.
