Question about the medical exam for OCS

Ok, I only have 2 things left for my package, the med exam and the CO interview. Well here is my questions...
I am an aircrewman in the Navy and I have a current long form flight physical with an EKG ext.. that expires in December. Do I need to do another physical for my application or can I use the one from Dec 02? What forms do you fill out to send in or do I just go see my flight surgeon and say I am appling for OCS? Will he know exactly what I need?

You’re physical has to be done with in 12 months, the most recent the better. As for the forms you have to include the SF 88 and the SF 93 with the package.
