For all of those who have gotten their PRK waivers this question is for you.
I am in the middle of putting together my app and I need my PRK waiver. My 6 months are up since I had the procedure done, so I called my Dr.'s office and asked to have my medical records sent to me so that I may submit them to my OSO. Today, I received them by mail. I opened it up and it was 4 pages of double sided chicken scribble! Is this what I need to submit for the waiver or is there more to it than this? I kind of thought I would be getting something like 20-30 pages long typed. Not hand written on a few sheets of paper.
Anyone who has applied for the waiver can you please help me out with this?
