When are you allowed to wear the sword? I want to imagine people standing duty in khakis with it in a pith helmet, but I get the impression that's not how it's done.

Swords are prescribed as part of Full Dress Blue (SDBs with big medals) and Full Dress White (chokers with big medals). Required items for O-4 and above, optional for O-3 and below.

Cutlasses. It's not an official uniform item, though there's a push to make them sword-equivalents for Chiefs.

You can also be an officer at the academy and stand OOW. Then you get to wear a sword all day when you walk around. Either that or it was just at formation time. I don't remember. But swords were used there more than anywhere else...
