Medical records no longer on file for DD2807 "yes" items + dental question

I'm preparing to put together an OCS package and I'm trying to obtain all my medical records related to anything I'll have to mark "yes" for on the DD2807 form (to my understanding, this is all I need to need to get records for, but please correct me if that's wrong). I've got a few questions about that.

1. The form asks about "A head injury, memory loss or amnesia" and "Have you ever been a patient in any type of hospital?". When I was about 7, I banged my head pretty hard on a coffee table and went to the ER (no concussion or anything serious), and when I was 1, I was in the hospital for a few days for pneumonia. I called the hospital both of these incidents took place at and I've sent them a release authorization so I can try to get my hands on those records, but they informed me they probably won't have them on file any more since they were both more than 10 years ago. If the records don't exist any more, is this going to make the process difficult and drawn out? Is there anything I can do to make it go more smoothly?

2. A few years ago I was in a car accident and I broke my nose. I had a CT scan (which showed no abnormalities) and stayed 1 night in the hospital. Does cartilage count as "Broken bone(s) (cracked or fractured)" or just actual bones? I'll still be including these records since it falls under 'head injury' and 'been a patient in a hospital', but I'm wondering if I should check 'broken bone' or not.

3. Lastly, is there somewhere I can view the requirements for passing the dental checks? I need a root canal so I'm sure I'll need to get that taken care of. Would that be immediately disqualifying or if I get selected, could I get it done before OCS and send follow-up documentation? And finally, would needing a root canal (due to decay) count as "Severe tooth or gum trouble" on the DD2807?

EDIT: Sorry, thought of one more question. All of my wisdom teeth were side impacted and I had them removed under general anesthesia a few years ago. Would that count as "Severe tooth or gum trouble"?

I'm sure you are fine. Nobody cares if you ever in your life went to the hospital/ER under pretty normal circumstances. They probably want to know if you broke your back in 15 different places, spent a year in traction, and have trouble standing up now.....or that you blew out (insert body part) playing college ball and you are basically physically broken for the rest of your life as a result. I had plenty of dumb little things like you mentioned, and I'm sure most everyone who didn't grow up in a bubble did too.

Ordinarily I wouldn't really be worried about the impact of the injuries themselves since they really were quite minor. I just wonder if MEPS will err on the side of caution without the supporting records, and they'll make things tricky because of it. Essentially something like "you seem fine and it was a very long time ago, but since we don't have the records, we can't be trusting enough to PQ you in case there was some complication we don't know about" (even though there wasn't, but with no records it would just be my word). I know I'm being paranoid, but I've read a ton of MEPS horror stories.

I was a ROTC dude, so I don't think I ever did MEPS (or maybe I did and didn't realize I was), but in general, you are going to have a lot of X-rays, a pretty thorough physical exam, and if you had some very serious pre-existing physical condition (like would be present from previous traumatic injury that has disabled you in some manner), they would notice it. I could be wrong, but I think you have nothing to worry about.

1. probably not an issue, sounds like parents being careful

2. you stayed a night in the hospital, that would be a yes under "have you been hospitalized" then they will ask why, then they will ask for records, so tell it like it is and let the doctor decide.

3. no info on this.

Thanks! I was definitely going to include #2 anyway because of the hospitalized question, I was just wondering if I should also check yes for 'broken bone' because of my nose, or if broken cartilage wouldn't necessarily qualify as a broken bone.

Also, can you confirm that the records only need to be brought for issues notated on the DD2807? Or do I need to track down all my medical records ever? The former makes more sense, but I just want to be sure so that I can get all this done ASAP.

MEPS will tell the NRD what records they want, the NRD will tell you, then you will provide.
