I thought this was going to be about a video game on Xbox 360 or PS3, after all, it is Christmas!! 
That is why you dont throw a grenade like a fastball. Sad to say, similar things happen at US ranges. The one I heard about most from instructors was students that would freeze once the pin was pulled and stand there holding the grenade instead of throwing it. Luckily there were measures in place when such things happened.
How do you throw one then, sorta toss it?
RB, then right stick to "look" where you want to throw it. Don't hold it too long.
It is a lobbing motion, except closer to a hookshot than a softball toss if that makes sense. (It starts behind your body) Much more controllable and more likely to clear close obstacles than a fastball pitch.

Navy guys teaching Navy guys the proper technique for throwing grenades... I don't know if I should laugh or cry...
Be sure to cook it off long enough that they can't throw it back.
…and that Chinese student runs back to formation to clean his trousers and tell his bros what just happened while that instructor sets the pit up for the next grenade toss as training continues.
US Marines on the other hand ->
Training is halted and multiple investigations are immediately started. The range control officer & SNCOIC as well as the company commander (who happened to be on two weeks baby leave during the incident) are all fired, anybody in the chain of command up to O-5 with a “commander” or “Sgt” in their billet description or rank are given shitty FITREPS and the grenade-throwing instructors are all NJP'd because they didn’t go “exactly by the reference material”.
TECOM immediately begins a new MarineNet course called “Global War on Error – Grenade Throwing” which discusses how to properly toss a grenade and all MOS’s & ranks O-5 and below are required to complete it annually, with a pass/fail multiple choice test given at the end of the course.
A couple weeks and numerous "man hours" are spent putting together “some slides” on the lessons learned from the grenade tossing mishap “for the boss” turns into a 58-slide PowerPoint brief and all commanders will have to incorporate it into the all-hands safety brief given each 72 & 96 as well as during the multiple safety stand-downs throughout the year.
Finally, HQMC will release an ALMAR stating all Marines must sign a statement of understanding on the proper procedures of grenade tossing before tossing a grenade and their OIC must sign a statement of understanding stating that that Marine has signed the statement of understanding on the proper procedure of grenade tossing before tossing the grenade.
Training can now continue...
Sidenote question - anybody know if the MCRD's are throwing grenades anymore? I know they had stopped for awhile. Talking about a shitty gig...those drill instructors who have to walk into the pit with a recruit holding a live grenade.

Laugh away, I have no clue how to throw one as I never have, I would've thought just like throwing a baseball. Is the lobbing motion correct?
I don't know what they teach now but I was taught to throw it overhand like a baseball. (or to do what ever you needed to do to get it close to the target.) M67 grenades are shaped like a baseball for that reason (so we were told). I only threw 5-6 live grenades but that was enough to figure out that the damn things are heavy. No matter how hard I tried it never seemed to go as far away as I wanted it to go. As far as "milking" or "cooking" a grenade before you throw it you might want to keep in mind that just like everything else, grenade fuses are made by the lowest bidder.
I threw a WP grenade once and it scared the hell out of me. It looks and is shaped just like a smoke cannister (except it's puke green). To me it seemed heavy AND somewhat awkward to throw. Not everyone could throw it farther than the casualty radius so we almost never practiced throwing them.
