To elaborate on the TW-5 story ... it was night and the Blue Brains CL says "Ldg Lights OFF" where as the NATOPS PCL says "Ldg Lights AS REQUIRED".
Common sense would dictate to leave the lights on as it was probably dark out but when you get into a habit pattern it's hard to break. This might have been his first night flight.
I GUARANTEE if he was used to seeing "AS REQUIRED" and then saying to himself "hey it's daytime I don't need the lights" when it was night time he would have used the same deductive reasoning to turn the lights on.
I HATED switching from the BB to the PCL but saw the reasoning and after a few flights it was no big deal.
