Premature canopy deployment at the World Freefall Convention in Quincy IL. Skydiver's main parachute deploys in the door, and wraps around the tail. Skydiver is towed for a while and cuts away. Otter is a very strong airplane and everyone survived unscathed.

That's good to hear.
 
Now, I don't know for fact, but I would be willing to bet a case of beers the guy hadn't checked his closing loop and it either broke or fell out when the wind hit.
