What information to bring [to MEPS] for PRK surgery

Hi guys/gals,

So far I have taken the ASTB and waited out the 6 month post-op for PRK eye surgery (also lost 40 lbs so I'm a bit chipper :) ). My recruiter says that I need to gather my 6 month post-op report then meet with him to fill out paperwork in regards to heading to MEPS.

My questions to you guys is what all do I need to request from my eye surgeon? Do I only need the 6 month check-up report or do I need other stuff? I remember reading somewhere about needing 1 month, 3 month, and 6 month reports but I can't find the report now for the life of me.

There is a prescreen form for meps that you have to fill out and list medical conditions, surgeries you've had or have. I had PRK done as well and I brought all pre op, op and my 1, 3 and 6 month post op. Bring as much as possible , you don't want to be short on paperwork when it comes to MEPS. Hope this helps.
