Refractive vision

Recently I visit my local OSO office about prk surgery. He informed me that I need to be within a certain bracket of refractive vision error to even have a chance for a waiver, though he didn't know what the numbers ranged from. Through my research I found that -8.00 and +3.00 were the limits for the waiver.

Now, my question... does the -8.00 refractive error limit account for the added sum of both eyes or is it the maximum for each eye?

My current contact prescription is (r/l) -4.00/-4.75.

There is an entire thread on PRK surgery in the Doc's corner forum that is a wealth of info, but the short answer is no. It's not a sum of refractive error for both eyes. I was -4.75 in each eye and got a waiver.
