Maybe that's what condition two is! (slide forward, no round in chamber, magazine with rearward facing rounds, weapon on safe).

Yup, that's it..I let some buddies at the range take a look. They couldn't see the mistake. Kinda surprised that the editors didn't catch this.

Condition II is round in chamber, hammer down. You don't see it in the M-16 because you can't have the hammer down with a round in the chamber.
