I have. On the fillable document provided for the 1420.1b there is no continuation sheet available. Also scanned the instruction.
What are the blocks? If it is the awards section, I put them in the wavier concerns section. Page 11 I believe
I don't think it matters. I listed 4 awards on the page 4 or whatever it is, and then I attached a copy of the citation for other awards that weren't listed ??♀️
