Navy Officer Recruiting Tattoo Screening Form 1131/27

Hello,

I'm currently active duty, working on a package for OCS-Pilot. I'm having a hard time on exact instructions and exactly how/whom to send the tattoo waiver form.

I have the Navy Officer Recruiting Tattoo Screening Certificate 1131/27 (Rev. 3-06).

It says if I answered yes to any of questions 1-5, that I need to do an interview with the CO or XO to determine waiver eligibility. If I'm eligible, I need to forward a picture / applicable PRODOCS to COMNAVCRUITCOM.

Tattoo in question is more than one quarter of body part, of which is on my back. Otherwise, it meets all other guidelines.

So beyond the actual interview, how exactly do I send this to COMNAVCRUITCOM

Just document the tattoos, include photos, and route to your XO for signature. They don't have to interview you, they just have to make sure the tattoo/s align with Navy regs.
