HELP - Need Clarfication on Exam Needed

After submitting my paperwork to MEPS and having the Navy Docs evaluate it, they came back with this...

"Provide current manifest refraction/Will need to get anther M.R. at least 1 month later on PUHLES/deferred"

So my question is what actual exam do I need or should I ask for when scheduling an appointment with my ophthalmologist ?
