Flight Surgeons Memo

I need one of these for my CNATRA Reserve Package (and any other units I have been looking at) and cannot find any info as to what the hell one is.

I'd rather go to a doc knowing what I need from them, vice asking one to write me a thing that I heard about in a message via email.

Any idea?

I think I have until mid may to get it in, but I'm also on a boat in Russia, so getting things stateside is not exactly quick.

It's just a note from the doc saying you're aeromedically qualified. Yes, that sounds like an up-chit, but this is just a memo, not a form.
