Was forwarded this e-mail today concerning my eye exam. I was selected for SNA and graduating in April.

Anyone seen or dealt with this before? Is this just a red-tape issue since it hasn't been accepted for review?

It sounds like someone jacked up the tests/forms. I would talk to the medical facility where you did the eye exam before and show them exactly what you've posted here. It seems like it should be fairly straight forward to correct, though the suspense time is relatively short, you may have to get this done quickly.
