Can any tell me what the deal is with the PRK check up forms for the 30, 60, and 90 day check ups. When I took the form in that my rec. sent me the assistants were surprised and said that they haven't seen one like that in a really long time. In fact, they couldn't even do some of the things on the form because they no longer have that kind of equipment. Out of date they say. I had to call my rec. in the chair and let the people talk to him to make sure they wanted to give me this cyclogel stuff (which turns out they don't use that anymore b/c it diolates your eyes for 3 days straight), but they didnt have to. Does they Navy have an updated form for these check ups and I just didn't get one? Will the "N/A" on some of the ?'s hurt me down the road?
