If you're determined to be NPQ'ed before going to OCS you would apply for the waiver. Now say you show up to OCS and a flight doc determines something is not right. Then you get to go through the process again.
For instance a buddy of mine had PRK and needed a waiver. He got it before he showed up to OCS. He had no medical problems at OCS. Then there is the large group of people that flight docs find with some sort of problem and need to get waivers.
Once you graduate OCS and check in to API you will be sent to NAMI. Unless you are due for a short/long form physical (not a commissioning physical) you will most likely get an admin upchit. This consist of turning your records in, having a doc review them and getting a piece of paper saying you are cleared to train.

You will see a flight doc at minimum of once a year in your aviation career. Your flight physical expires on the last day of your birth month. The only things that change are the medical standards by which you are held. (OCS SNA>SNA>NA)
